You will be required to submit following documents along with duly filled Customer Application form (CAF) for taking an MTNL prepaid or postpaid connection.

  • Self-attested Photograph
  • Proof of Identity
  • Proof of Address

  • Arms License
  • Voter ID Card
  • CGHS/ECHS Card
  • Certificate of address having a photo, issued by MP/MLA/Group A Gazetted Officer in the letterhead.
  • Certificate of address with a photo from Govt. Recognized educational institutions (for students only)
  • Certificate of address having photo issued by Village Panchayat /Head (For rural areas)
  • .Address card with photo issued by Dept. of Posts as Proof of Identity & Address
  • Current passbook of Post Office/Any Scheduled Bank as Proof of Address, with photo as Proof of Identity & Address (Not older than 3 months).
  • Aadhar(UID) card
